Relative efficacy of catheter ablation vs antiarrhythmicdrugs in treating premature ventricular contractions: Asingle-center retrospective study

Summary

BACKGROUND

It isunknownwhetherradiofrequencyabla-
tion (RFA)orantiarrhythmictherapyissuperiorwhentreating
patients withsymptomaticprematureventricularcontractions
(PVCs).

OBJECTIVE

To determinetherelativeefficacy ofRFAandantiar-
rhythmic drugs(AADs)onPVCburdenreductionandincreasingleft
ventricular systolicfunction.

METHODS

Patients withfrequentPVCs(41000/24 h)weretreated
either byRFAorwithAADsfromJanuary2005throughDecember
2010. Datafrom24-hourHoltermonitoringandechocardiography
before and6–12 monthsaftertreatmentwerecomparedbetween
the 2groups.

RESULTS

Of 510patientsidentified, 215(40%)underwentRFA
and 295(60%)receivedAADs.ThereductioninPVCfrequencywas
greater byRFAthanwithAADs(–21,799/24 hvs –8,376/24 h; Po
.001). Theleftventricularejectionfraction(LVEF)wasincreased
significantly afterRFA(53%–56%; P o .001) butnotafterAAD
(52%– 52%; P ¼ .6) therapy.Of121(24%)patientswithreduced
LVEF, 39(32%)hadLVEFnormalizationto50%orgreater.LVEFwas
restored in25of53(47%)patientsintheRFAgroupcomparedwith
14 of68(21%)patientsintheAADgroup(P ¼ .003). PVCcoupling
interval lessthan450ms,lessimpairedleftventricularfunction,
and RFAwereindependentpredictorsofLVEFnormalization
performed byusingmultivariateanalysis.

CONCLUSION

RFA appearstobemoreeffectivethanAADsinPVC
reduction andLVEFnormalization.
