Amiodarone and Lignocaine in Shock Refractory VF and Pulseless VT

Kudenchuk et al report the outcome of a randomized clinical trial of amiodarone/lignocaine v placebo for out of hospital cardiac arrest secondary to shock-refractory and pulseless ventricular tachycardia. Drug therapy was not associated improved survival or survival with good neurological outcome. The editorial is available here. The editorial (as the paper does) speculates on reason the trial was negative (including underpowered). However, they note no clinically meaningful difference was seen and emphasize the importance of bystander CPR in the “chain of survival” (10% absolute increase in survival).
